Skilled home health care includes a broad range of services such as nursing, therapy, and medical management for an illness or injury. It’s less expensive and more convenient than care received in hospitals and nursing centers. Studies show that seniors recover more quickly when they stay in a familiar environment, sleep in their own beds, and keep their established routines. For seniors who are chronically ill, disabled, or recovering from short term illness or surgery, skilled home health care may be the answer.

To qualify for skilled home health care, a patient must be under the care of a physician who orders this specific care and meet certain criteria. We provide home health in Tampa, Sarasota, and Clearwater under licenses for AHCA Regions 5, 6, and 8.

  • A doctor must prescribe home health care for you or your loved one.
  • You need either nursing care or therapy services such as physical, occupational and/or speech therapy on an intermittent basis.
  • You must be restricted in your ability to leave home, which is called “home-bound”, and your homebound status must be certified by a physician.

We can provide a wide range of services through our home health agency and its affiliated companies. Home health is governed by Medicare and your specific Medicare Advantage program. On the first visit, a nurse or therapist will conduct an initial evaluation. At Angels Care Home Health, this thorough interview and evaluation is part of our coordinated approach to managing your overall health status. Our evaluation focuses on educating you in self-care management and partners with your doctor to promote disease prevention and proactive care – which includes the family and/or caregivers.

This team approach facilitates an easier transition for you from a hospital or nursing facility to your home, and actively engages you and your caregivers in your health care.

Yes, we will communicate with your doctor to discuss the assessment and work together to develop your personal plan of care. Angels Care Home Health employees will implement your physician-ordered plan of care and keep your doctor updated about your progress. If your condition or needs change, we will collaborate with your doctor to review your plan of care and make any adjustments deemed necessary.

The frequency of home health care visits and the services provided are based on your doctor’s orders in your personal plan of care. Your doctor may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best home health care for your needs. At Angels Care Home Health, we’ll communicate with you any and all changes to your visit schedule along with your plan of care.
